HOW TO FILL OUT THE MAJOR PROGRAM FORM

Most of the major program form is straightforward. Below are the answers to a couple of regularly asked questions. If you have other questions, please call or email Dr. Doty for assistance.

Arts and Sciences Major Program Form

  • The question, "Have you filed a degree for application?" should have an answer of "NO". This is a prompt to remind you that you need to fill out your degree for application in the College Offices in Denney Hall.
  • Your GEC's do NOT go on this form
  • OPTION SPECIFIC ELECTIVES refers to the courses that you take for your OPTION (A,B,C,D,E, or F) that are NOT Physics courses. ADDITIONAL REQUIRED PHYSICS refers to the Physics courses that you took for your option that are NOT part of the listed CORE REQUIRED PHYSICS. A list of these courses can be found in the table at: http://www.physics.ohio-state.edu/undergrad/bs_physics.php
  • Remember that ALL grades on the major program form MUST be above "C-".
  • This form, when complete and signed, will go to the College offices in Denney Hall OR to your Honors advisor.

Engineering Physics Major Program Form

Typically very few questions are asked about this form, however, please make sure you complete BOTH the Major Program Form and the Application for Degree from the College of Engineering.

Please e-mail Dr. Sandy Doty, dotys@mps.ohio-state.edu to make an appointment to have the major program form checked and signed. Any petitions for course substitutions will be completed at or BEFORE this time. After checking your major program form, an exit interview will be conducted. This will take approximately 30 minutes. You may also be contacted by the Undergraduate Studies office, after completing your paperwork, for further details about your postgraduation plans.
